架关键参数初探Combined periodontal-pulp lesions involve pulp tissue, periapical tissue and periodontal tissue at the same time, which are relatively complicated in diagnosis, treatment and prognosis judgment, and are common clinical difficulties[1]。 Large range of periapical lesions is not conducive to the control of pulp and periapical infection, and periodontal bone destruction of affected teeth is also not conducive to the prognosis of affected teeth. Therefore, clinically, the treatment strategy of extraction and restoration is often adopted for this kind of affected teeth with severe periodontal-pulp combined lesions. Stomatological Hospital of Peking University School of Stomatology admitted a complicated case of combined periodontal-pulp lesions with large periapical bone defects connected with periodontal bone defects. After root canal treatment, microapical surgery, basic periodontal treatment, guided tissue regeneration (GTR) + bone grafting and other multidisciplinary treatments, the periapical and periodontal tissues of the affected teeth were significantly recovered, and the short-term curative effect was good, which can provide reference for the solution of similar clinical problems. Combined periodontal-endodontic lesions are common in clinic, and their etiology and clinical trend are complex and changeable. There are still many controversial issues in the diagnosis, selection of intervention procedures and treatment effect. The clinical diagnosis and treatment of this lesion highly requires the cooperation between different disciplines. This article expounds and discusses the diagnosis and treatment process of a complicated case of combined periodontal-pulp lesion with large-scale apical bone defect and periodontal bone defect, demonstrates the original intention and rational persistence of both doctors and patients to preserve natural teeth as much as possible, as well as the perfect clinical technical details, and provides reference for the diagnosis and treatment of related clinical cases for stomatologists.
