Expert Introduction: Professor Ye Zhaoxiang of Tianjin Medical University Cancer Hospital believes that with the wide application of neoadjuvant chemotherapy in breast cancer, the toxicity of liver and heart caused by chemotherapy has received more and more attention. Professor Zhang Jiuquan's team found that the prevalence of liver chemotherapy-related steatitis was as high as 42.8%; It was also found that the epicardial fat volume increased significantly in patients receiving chemotherapy. In another study, the team quantified the degree of myocardial fibrosis in breast cancer patients treated with anthracyclines by extracellular volume (ECV) of cardiac myocardium, and found that myocardial ECV gradually increased from baseline to 3, 6, and 12 months after treatment, and the increase of myocardial ECV in patients with cancer treatment-related cardiac insufficiency was higher than that in patients without cardiac insufficiency. Professor Liu Zaiyi of Guangdong Provincial People's Hospital believes that how to deeply understand the toxic and side effects of tumor treatment through imaging has become an important issue faced by tumor imaging researchers. Zhang Jiuquan's team put forward new insights in the imaging evaluation of liver, heart and nervous system toxicity caused by chemotherapy. Firstly, through MRI quantification of liver fat and CT quantitative analysis of epicardial fat volume, it was found that the incidence of liver chemotherapy-related steatitis and epicardial fat volume in breast cancer patients receiving neoadjuvant chemotherapy were significantly higher than those in the non-treatment group, and there was a significant correlation between them. Then, ECV was obtained by chest CT to quantify the degree of myocardial fibrosis in breast cancer patients undergoing chemotherapy, and it was found that ECV increased significantly in patients with cardiac insufficiency. In the nervous system, breast cancer patients can develop extensive morphological changes in the cerebral cortex after completion of the first cycle of neoadjuvant chemotherapy. These brain regions are closely related to cognitive domains such as memory, executive function, attention and information processing speed. These studies have important reference significance for carrying out comprehensive tumor imaging research.
