This issue focuses on the research progress of orbital disease and eye plastic surgery. Professor Li Dongmei is the responsible editorial committee responsible for the planning, composition and review of the key contents of this issue.
Thyroid-associated ophthalmopathy (TAO) is now the most prevalent orbital disease in adults, with a complex pathophysiology and a wide range of clinical symptoms. In recent years, significant progress has been made in the research on the treatment of TAO. Periorbital steroid injections are considered a precise local treatment and can improve ocular symptoms with minor side effects. However, there is no standardized orbital injection protocol in China, resulting in increased complications. Therefore, it is essential to standardize periorbital steroid injections in treating TAO.
Thyroid-associated ophthalmopathy (TAO) is an orbital disease closely related to thyroid diseases. TAO has a prolonged course and possibility of blindness and disability. Its diagnosis and treatment can be complex with multiple disciplines involved. To improve the standardized diagnosis and treatment of TAO in China, the Oculoplastic and Orbital Disease Group of Chinese Ophthalmological Society of Chinese Medical Association and the Thyroid Group of Chinese Society of Endocrinology of Chinese Medical Association have jointly drawn up this first clinical guideline in China for TAO by complying with the principles, methods and steps of guideline formation. In this guideline, there are 24 recommendations for the major clinical problems in the diagnosis and treatment of TAO, including the clinical diagnostic criteria, disease staging and grading methods, novel treatment based on the pathogenesis, multi-disciplinary treatment, risk factor control, individualized selection of treatment such as drugs, radiation and surgery, therapeutic efficacy evaluation and critical illness management. This guideline provides instructions for clinicians to carry out relevant clinical and research work.
To evaluate the efficacy and safety of maximal orbital decompression surgery and intravenous glucocorticoid (ivGC) pulse therapy as the initial treatment for dysthyroid optic neuropathy (DON).
Retrospective cohort study. Forty-three patients (66 eyes) with DON who were treated during April 2019 and November 2021 in Peking University People′s Hospital were included. According to the treatment methods, they were divided into the glucocorticoid group (21 cases, 33eyes) and the surgery group (22 cases, 33eyes). The glucocorticoid group received a total dose of 7.5 g ivGC therapy, while the surgery group received maximal orbital decompression surgery. Clinical outcomes were recorded and assessed including pre-and post-treatment logarithm of the minimum angle of resolution best corrected visual acuity (logMAR BCVA), clinical activity score (CAS), visual evoked potential (VEP)-P100 wave amplitude and peak time, muscle index (MI), and clinical proptosis. Adverse effects were also recorded. Follow-up data at 6 months after surgery or ivGC therapy were used as the post-treatment data. Wilcoxon signed rank analysis, independent or paired sample t test, Mann-Whitney U test, Chi-square test were used for statistical analysis. Non-normal distribution data were expressed in M(IQR).
There was no significant difference (all P>0.05) between the two groups in terms of gender, age, course of disease or laterality, as well as baseline data such as pre-treatment BCVA, CAS, VEP-P100 wave amplitude and peak time, and MI. After treatment, logMAR BCVA of the glucocorticoid group increased significantly from 1.00 (0.65) to 0.70 (0.60), (Z=-4.55), while BCVA of the surgery group increased significantly from 1.00 (0.95) to 0.60 (0.55), (Z=-4.87). The post-treatment CAS of the glucocorticoid group decreased significantly from 5 (2) to 2 (2) (Z=-4.91), while that of the surgery group also decreased significantly from 5 (2) to 2 (2) (Z=-4.94). After treatment, the VEP-P100 wave amplitude of the glucocorticoid group increased significantly from (3.49±1.34) μV to (5.26±2.00) μV (t=-9.08), while that of the surgery group increased significantly from (3.21±1.74) μV to (4.78±2.36) μV (t=-9.24). After treatment, the P100 wave peak time of the glucocorticoid group was significantly shortened from (110.52±12.11) ms to (103.53±6.67) ms (t=4.67), while that of the surgery group was significantly shortened from (118.48±20.16) ms to (106.45±10.57) ms (t=2.84). There were significant differences between inner-group before and after treatment data (all P<0.001), whereas the inter-group after treatment data have no significant difference (all P>0.05). The orbital apex crowding relief (MI≤0.52) rate was 64% (21/33) in the glucocorticoid group and was 91% (30/33) in the surgery group, while the proportion of proptosis reduction (more than 2 mm) in the glucocorticoid group and the surgery group was 64% (21/33) and 94% (31/33), respectively. Surgery was better than ivGC therapy both in terms of orbital apex crowding relief and proptosis reduction (χ2=5.52, 7.34; both P<0.05). Serious adverse effects or complications occurred in none of the two groups. Mild adverse effects occurred in 38% (8/12) of the patients receiving glucocorticoids and in 59%(13/22) of the patients receiving surgical treatment, with no significant difference (P>0.05).
Both maximal orbital decompression surgery and ivGC therapy can effectively improve the visual function and reduce the CAS of DON with high safety. Meanwhile, surgery has more advantages in relieving orbital apex crowding and reducing proptosis.
To classify the eyelid divided nevi using a new classification method, and to observe the distribution characteristics and surgical effects of these nevi.
It was a retrospective case series study. Thirty-nine cases (39 eyes) of eyelid divided nevus treated at Beijing Tongren Hospital affiliated to Capital Medical University were collected between October 2015 and January 2020. A new classification method, namely traditional classification combined with aesthetic unit classification, was used to classify eyelid divided nevi. According to different classifications, the corresponding surgical treatment methods were adopted. The distribution characteristics of different types of eyelid divided nevi were summarized, as well as the effects of plastic surgery.
Thirty-nine patients were affected unilaterally, including 26 females (67%) and 13 males (33%), aged (31.1±17.8) years. There were 5 cases (13%) of localized, 6 cases (15%) of a moderate range and 2 cases (5%) of a large range medial eyelid divided nevus respectively. There were 6 cases (15%) of localized and 3 cases (8%) of a moderate range central eyelid divided nevus, respectively. There were 4 cases (10%) of localized and 6 cases (15%) of a moderate range lateral eyelid divided nevus, respectively. There were 2 cases (5%) of a moderate range and 5 cases (13%) of a large range total eyelid divided nevus, respectively. Pathological results revealed 20 cases of intradermal nevus (51%), 18 cases of compound nevus (46%) and 1 case of boundary nevus (3%). After 6 months to 2 years of observation, the patients′ appearance was satisfactory. The treatment results were excellent in 36 cases (92%) and good in 3 cases (8%). Even though the pigmented nevus at the eyelid margin recurred in two cases (5%), there was no recurrence after reoperation.
Medial and moderate-range eyelid divided nevi are more common, and the traditional classification combined with aesthetic unit classification is more accurate for the classification of eyelid divided nevi. Different types of eyelid divided nevi can be treated by corresponding surgical methods with satisfactory results.
To investigate the clinicopathological features of adult orbital xanthogranulomatous disease (AOXGD) with lacrimal gland reactive lymphoid hyperplasia.
Retrospective case series study. The clinical and pathological data of AOXGD cases diagnosed and treated in Tianjin Eye Hospital from January 2002 to December 2021 was reviewed, and the clinical characteristics, radiologic findings and pathological characteristics of periocular and lacrimal gland lesions of 5 cases were retrospectively analyzed. The expression of IgG4 and IgG protein in periocular and lacrimal gland lesions was detected by immunohistochemical staining, and the role of IgG4 in AOXGD was preliminarily studied.
There were four females and one male with an average age of 53.8 years (39 to 77 years). Among the five AOXGD cases, there were three cases of adult-onset xanthogranuloma, one case of adult-onset asthma and periocular xanthogranuloma, and one case of necrobiotic xanthogranuloma. All cases involved both eyes. The swelling of eyelids was observed in five cases, and the yellow or pale yellow eyelid skin was found in two cases. Imaging examinations showed the tumor mainly involved the eyelids, subcutaneous tissues, anterior orbit and lacrimal gland. A large number of foam cells and typical Touton giant cells were found in the periorbital lesions, accompanied by different degrees of fibrosis. The fibrinoid necrosis was detected in one case of necrobiotic xanthogranuloma. The lacrimal gland lesions showed different types of reactive lymphoid hyperplasia, including IgG4-related disease in two cases, follicular lymphoid hyperplasia in two cases and focal lymphoid hyperplasia in one case. IgG4 levels of periorbital and lacrimal gland lesions were elevated in four cases. Asthma and elevated serum IgG4 were found in one case of adult-onset periocular xanthogranuloma. Three patients underwent surgical resection and adjuvant hormone or immunosuppressive therapy, and two patients underwent simple surgical resection. The patients were followed up for 1.5 to 10.0 years, one patient was lost, and four patients had no recurrence.
AOXGD with lacrimal gland reactive lymphoid hyperplasia is a group of rare diseases. The periorbital lesions of that are characterized by proliferation of foamy histiocytes and Touton giant cells, and the lacrimal gland lesions of that manifest as IgG4-related disease in some cases.
To observe the restriction effect of silicone oil retention sutures and their protective effect on the corneal endothelium.
Retrospective case series study. The clinical data of 6 patients (6 eyes) with lens-iris damage after severe ocular trauma were collected from March 2017 to November 2021 in Beijing Tongren Hospital. Their was combined retinal injury in each eye, so silicone oil retention sutures and silicone oil filling were performed. The silicone oil position was observed by slit lamp and ultrasound biomicroscopy. The corneal endothelium was examined by specular microscopy during the follow-up.
All patients were male, with an average age of 47 years (range, 26 to 73 years). The diseased eyes included 2 left eyes and 4 right eyes. Five eyes had ocular rupture, and 1 eye had ocular contusion. The follow-up time ranged from 5 months to 51 months, with an average of 18.5 months. In all 6 eyes, the silicone oil was completely located in the posterior segment and never touched the corneal endothelium. Four eyes maintained healthy and had a clear cornea during the follow-up, while 2 eyes developed band keratopathy at the 11 month and 16 month separately during follow-up, 1 of which was found to suffer corneal decompensation at the last follow-up (51 month). Both band keratopathy and corneal decompensation were caused by severe anterior segment trauma and were not related to the silicone oil contact with the cornea. There were no complaints about occlusion or glare due to silicone oil retention sutures. All patients did not need to keep the prone position postoperatively. Silicone oil emulsification occurred in 1 eye and ocular hypertension occurred in this eye after silicone oil emulsification.
Silicone oil retention sutures can effectively prevent silicone oil from migrating to the anterior chamber or contacting the corneal endothelium.
To evaluate the efficacy and safety of superior rectus transposition (SRT) with/without augmented suture and vertical rectus transposition (VRT) for the treatment of strabismus caused by complete abducens nerve palsy.
This was a retrospective cohort study. Forty-two patients (42 eyes) with complete abducens nerve palsy underwent strabismic surgeries from January 2015 to November 2020 in Tianjin Eye Hospital. According to the different procedures, the patients were divided into three groups: SRT group (16 cases, SRT with medial rectus recession), superior rectus transposition with augmented suture (SRTA) group (13 cases, SRT with Buckley suture and medial rectus recession) and VRT group (13 cases). The preoperative and postoperative (1, 6 and 12 months) data including deviations, ocular motility, binocular vision and surgical complications among three groups were analyzed and compared. χ2 test was used for comparison of count data among three groups. The measurement data were compared among three groups by the repeated measures ANOVA. LSD-t test was used for within-group comparison and between-group comparison.
There was no difference in sex ratio, age and course of disease among the groups (all P>0.05). The horizontal deviations of the three groups at 1, 6 and 12 months after surgeries was lower than that before surgeries, and the difference was statistically significant (all P<0.001). The horizontal deviations of the SRT group, SRTA group and VRT group at 12 months after surgeries were (+0.8±5.8), (+0.8±4.5), (+1.2±2.5) prism diopters (PD), respectively, lower than that of the preoperative (+82.8±17.2), (+77.7±26.1), (+71.5±18.6) PD. However, there was no significant difference among different postoperative follow-up timepoints (all P>0.05). There was no difference in horizontal deviations before surgeries and at 1, 6 and 12 months after surgeries among three groups (P>0.05). There were significant differences in the scales of abduction motility among preoperative, postoperative 1, 6 and 12 months measurements for three group (all P<0.001). The scales of abduction before surgeries in the SRT group, SRTA group, and VRT group were (-4.4±0.5), (-4.4±0.5), (-4.5±0.5) scale and at 12 months after surgeries were (-2.3±0.7), (-2.2±0.5), (-2.1±0.6) scale respectively. But there was no change among different postoperative follow-up timepoints (all P>0.05). Preoperative and postoperative 1-, 6-and 12-month abduction motility was similar among three groups (P>0.05). There were significant differences in the scales of adduction limitation among preoperative, postoperative 1-, 6-and 12-months measurements for three group (all P<0.05). But there was no change among different postoperative follow-up timepoints (all P>0.05). There were significant differences between the SRT group [(-0.9±0.6), (-0.8±0.6) scale] and the SRTA groups [(-1.5±0.5), (-1.4±0.5) scale] (t=-2.62, -2.52) and between the SRTA group and the VRT group [(-0.8±0.8), (-0.6±0.7) scale] (t=2.62, 3.01) at 6 and 12 months after surgeries (all P<0.05). The outcomes of binocular vision at postoperative 12 months were similar among three groups (P>0.05). No patient had torsional diplopia and anterior segment ischemia. Only 2 patients from the SRTA group had hypotropia of 4 to 5 PD in the primary position associated with supraduction limitation.
SRT with/without augmented suture and VRT are effective and safe procedures for the treatment of strabismus caused by complete abducens nerve palsy. They could correct deviations, improve abduction motility and restore binocular vision, with stable outcomes and a small risk of vertical and torsional diplopia.
A total of 170 consecutive patients (181 eyes) who were diagnosed with primary angle-closure glaucoma (PACG) and underwent phacoemulsification and intraocular lens implantation (PEI) combined with goniosynechialysis (GSL) with an instrument under a gonioscope in the Eye Hospital, Wenzhou Medical University from January 1, 2017 to December 31, 2019 were observed. Anterior chamber angle synechiae of 0 (0, 2) clock hour were released after PEI, while after combined goniosynechialysis, synechiae of 6 (3, 8) clock hours were further released. No matter in chronic PACG and acute PACG with cornea edema, the extent of anterior chamber angle synechiae was different before surgery and after PEI, and so was it in post-PEI and PEI-GSL prospectively (all P<0.01). Phacoemulsification alone can reopen the synechial anterior chamber angle in both chronic PACG and acute PACG in part, but not sufficiently. Combined goniosynechialysis under a gonioscope with an instrument is essential and effective.
A 19-year-old patient with bilateral eyelid swelling for more than 2 years visited the Department of Ophthalmology. Physical examination disclosed a moderately active mass in the upper eyelid and lacrimal gland area in each eye. Orbital MRI showed that the lacrimal glands were significantly enlarged and moved forward in both eyes, and the enlargement was more severe in the left eye. The patient was admitted twice to the hospital for removal of orbital masses and was diagnosed as orbital Rosai-Dorfman disease by histopathological examination. During three months of follow-up, the patient was in good physical condition, and there was no tumor recurrence.
A 20-year-old girl came to the Tianjin Medical University Eye Hospital because of progressive herniation of her right eye for 3 months. Examination revealed a huge mass behind the right eyeball, which was clinically diagnosed as a space-occupying lesion of the right orbit. Because the tumor was close to the nerve, most of the tumor was resected and pathologically diagnosed as a poorly differentiated leiomyosarcoma. Orbital evisceration was performed later. There was no recurrence during the 10-year follow-up.
A 52-year-old patient was initially diagnosed as hypophosphatemic osteomalacia in the Department of Endocrinology due to knee, foot and lumbosacral pain. The symptoms were not significantly relieved after phosphorus and vitamin D supplementation. Later, the imaging examination showed an orbital tumor in the right eye. The tumor was surgically removed, and the symptoms of systemic bone pain were relieved.
A patient complained of vision loss of his left eye which was crushed by iron ore for 11 months. The cornea of the injured eye was thin and swollen, and a large amount of rust-like material was observed to be deposited. An intraocular foreign body was found by orbital CT. During vitrectomy, a piece of metal sheet was found near the ora serrate, and the intraocular structure was severely damaged, and characterized by vitreous brown turbidity, a white optic disc, occlusion of blood vessels in the fundus, and peripheral retinal atrophy with degeneration. The patient was diagnosed as ocular siderosis in the left eye.
A patient presented with a chief complaint of a conjunctival mass in the right eye for 5 days. The slit-lamp examination revealed a reddish oval mass, about 3 mm in diameter, in the nasal conjunctiva of the lower central fornix, with a clear boundary, smooth surface, toughness by touch, good activity, no bleeding, and no tenderness. The tumor was excised completely. Solitary fibrous tumor of the conjunctiva was diagnosed by pathological examination.
A 6-year-old girl had binocular vision loss with pain for one week. The patient presented with symptoms such as non-communication, language deterioration, dysphonia, and choking when drinking and eating during the course. The serum myelin oligodendrocyte glycoprotein antibody was positive. Both the serum and cerebrospinal fluid anti-N-methyl-D-aspartate receptor antibody were also positive. The diagnoses were myelin oligodendrocyte glycoprotein antibody positive optic neuritis and anti-N-methyl-D-aspartate receptor encephalitis. High-dose intravenous glucocorticoids were given. Recurrence was not observed during the 15-month clinical follow-up.
The patient was a 29-year-old female. He complained of foreign body sensation in his right eye for half a year and went to Beijing Tongren Eye Center, Beijing Tongren Hospital affiliated to Capital Medical University. Ocular examination: binocular visual acuity was 0.8, intraocular pressure was 14 mmHg (1 mmHg =0.133 kPa) in the right eye and 16 mmHg in the left eye; The eyelashes of the lower eyelids of both eyes grew upside down, touching the cornea, and there was a blood vesicle below the nose of the cornea of the right eye, in which the plane of bloody fluid was visible (
The patient was a 60-year-old male. Because of blurred vision in his left eye for 1 year, he went to the Department of Ophthalmology, Affiliated Hospital of Jining Medical College. No previous systemic diseases such as diabetes and hypertension. Eye examination: visual acuity was 0.8 for the right eye and 0.25 for the left eye; IOP was 15 mmHg (1 mmHg =0.133 kPa) in the right eye and 13 mmHg in the left eye; There is no conjunctiva congestion and edema in both eyes, the cornea is transparent, the anterior chamber depth is normal, the pupil diameter is about 2.5 mm, and it is sensitive to light. The lens of the right eye is mildly opaque, and the posterior subcapsular cortex of the left eye is several flaky opaque with branches of varying sizes, like corals (Quintessential Picture 1); Fundus examination showed no abnormalities. Clinical diagnosis: Age-related cataract (posterior subcapsular type) in the left eye.

Ophthalmic surgery has the characteristics of high precision, small operating range, need of microscopic vision, and sensitive feedback, which put forward special requirements for surgical robots. The da Vinci robot, intraocular robotic interventional surgical system, hand-held micromanipulator, and co-manipulation robot have been developed and applied in various ophthalmic surgeries. These surgical robots have shown the advantages of improving the operation accuracy, stability, and dexterity in the ocular surface, cataract, and retinal surgeries.
Optical coherence tomography angiography (OCTA) is a new vascular imaging technology based on high-resolution optical coherence tomography image analysis. It can scan the moving red blood cells in blood vessels for three-dimensional imaging of human retinal and choroidal vessels. Since the close connection of vascular endothelial cells of the blood-retinal barrier is similar to that of the blood-brain barrier, the role of OCTA in the research of pathogenesis and course monitoring of a variety of central system diseases such as Alzheimer′s disease, Parkinson′s disease, stroke, migraine, multiple sclerosis and optic neuromyelitis has been widely discussed. This article reviews the application and progress of OCTA in central nervous system diseases.
